1. Acceptance of Terms

By accessing and using PatientBook ("Service"), you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by the above, please do not use this service.

PatientBook is a healthcare management platform designed specifically for medical professionals to manage patient information, appointments, and medical records securely and in compliance with all applicable healthcare regulations.

2. Use License

Permission is granted to temporarily download one copy of the materials (information or software) on PatientBook for personal, non-commercial transitory viewing only. This is the grant of a license, not a transfer of title, and under this license you may not:

  • Modifying or copying the materials
  • Using the materials for any commercial purpose or for any public display
  • Attempting to decompile, reverse engineer, or disassemble any software contained on the Service
  • Removing any copyright or other proprietary notations from the materials
  • Transferring the materials to another person or "mirroring" the materials on any other server

3. Healthcare Compliance & User Responsibilities

3.1 Healthcare Professional Requirements: PatientBook is intended for use by licensed healthcare professionals. Users must maintain valid medical licenses in their jurisdiction and comply with all applicable healthcare regulations.

3.2 Patient Privacy and Confidentiality: You acknowledge that patient information stored on PatientBook constitutes Protected Health Information (PHI) under HIPAA and similar regulations. You agree to:

  • Maintain strict confidentiality of all patient data
  • Only access patient records for legitimate treatment, payment, or healthcare operations purposes
  • Never share patient information with unauthorized third parties
  • Report any suspected data breaches immediately to PatientBook support
  • Comply with all patient consent requirements before sharing data
  • Maintain audit logs of all patient record access for compliance purposes

3.3 Informed Consent: You must obtain appropriate informed consent from patients before storing their medical information in PatientBook. Patients must be informed about data collection, storage, and usage practices.

3.4 Medical Accuracy: You are solely responsible for the accuracy, completeness, and appropriateness of all medical information and diagnoses entered into PatientBook. PatientBook is a record-keeping tool and does not provide clinical decision support or diagnosis recommendations.

4. Disclaimer of Warranties

The materials on PatientBook are provided on an 'as is' basis. PatientBook makes no warranties, expressed or implied, and hereby disclaims and negates all other warranties including, without limitation, implied warranties or conditions of merchantability, fitness for a particular purpose, or non-infringement of intellectual property or other violation of rights.

PatientBook does not warrant or make any representations concerning the accuracy, likely results, or reliability of the use of the materials on its Internet web site or otherwise relating to such materials or on any sites linked to this site.

Important: PatientBook is not a substitute for professional medical judgment or consultation. The Service is designed to facilitate record-keeping and should not be relied upon as the sole basis for clinical decision-making.

5. Limitations of Liability

In no event shall PatientBook or its suppliers be liable for any damages (including, without limitation, damages for loss of data or profit, or due to business interruption,) arising out of the use or inability to use the materials on PatientBook, even if PatientBook or an authorized representative has been notified orally or in writing of the possibility of such damage.

Clinical Liability: PatientBook is not responsible for any clinical errors, misdiagnosis, or adverse patient outcomes resulting from the use or misuse of the platform. Healthcare professionals remain solely responsible for all clinical decisions and patient care.

6. Accuracy of Materials

The materials appearing on PatientBook could include technical, typographical, or photographic errors. PatientBook does not warrant that any of the materials on the Service are accurate, complete, or current. PatientBook may make changes to the materials contained on the Service at any time without notice. However, PatientBook does not make any commitment to update the materials.

7. Materials License & Intellectual Property

PatientBook grants you a limited license to reproduce and distribute material from the Service, but only for the purpose of healthcare record-keeping and with proper attribution. This license does not include the right to:

  • Sell or rent any material from PatientBook
  • Reproduce material on any other website or in a networked computer environment
  • Remove copyright and other proprietary notices from materials
  • Transfer the license to another person

All content, features, and functionality of PatientBook are owned by PatientBook or its content suppliers and are protected by international copyright, trademark, and other intellectual property laws.

8. Links

PatientBook has not reviewed all of the sites linked to its website and is not responsible for the contents of any such linked site. The inclusion of any link does not imply endorsement by PatientBook of the site. Use of any such linked website is at the user's own risk.

If you find any links on PatientBook that are inappropriate or point to healthcare misinformation, please notify us immediately so we can evaluate and remove such links.

9. Modifications

PatientBook may revise these terms of service at any time without notice. By using this website, you are agreeing to be bound by the then current version of these terms of service. We will notify users of any material changes to these Terms via email or prominent notice on the Service.

10. Governing Law

The materials appearing on PatientBook are governed by and construed in accordance with the laws of the jurisdiction in which PatientBook operates, excluding its conflicts of law provisions.

Use of PatientBook constitutes acceptance of the terms and conditions set forth in this agreement. If you do not agree to these terms and conditions, you should not use this website.

11. Data Retention & Deletion

You are responsible for maintaining backups of patient data stored in PatientBook. While we implement industry-standard backup procedures, we strongly recommend that you also maintain independent copies of all critical patient information.

Data retention is governed by applicable healthcare regulations including HIPAA minimum retention requirements. Users must retain patient records for the period required by law in their jurisdiction.

Upon account termination, PatientBook will securely delete your data after the legally required retention period has elapsed, unless you request otherwise or applicable law requires longer retention.

12. Account Termination

PatientBook reserves the right to terminate or suspend your account immediately, without prior notice or liability, for any reason whatsoever, including if you breach the Terms of Service.

Upon termination, your right to use the Service will immediately cease. You will remain liable for all fees and charges incurred prior to termination.
